Interdisciplinary team

The team a PACE organization must maintain for each participant under 42 CFR 460.102, responsible for assessing the participant, developing the plan of care and delivering or arranging every service in it. Its members span primary care, nursing, social work, therapy, nutrition, activities, home care and transportation.

The interdisciplinary team is the unit of decision in PACE. Under 42 CFR 460.102 each PACE organization must establish one, composed at a minimum of a primary care provider, a registered nurse, a master’s-level social worker, a physical therapist, an occupational therapist, a recreational therapist or activity coordinator, a dietitian, a PACE center manager, a home care coordinator, a personal care attendant and a driver. The team is responsible for the initial assessment, the periodic reassessments, the plan of care, and the coordination of 24-hour care delivery. Services are provided on the team’s decision, and the organization pays for what the team decides.

For the initial comprehensive assessment the regulation names the eight members who must each evaluate the participant in person and write a discipline-specific assessment: the primary care provider, the registered nurse, the social worker, the physical therapist, the occupational therapist, the recreational therapist or activity coordinator, the dietitian and the home care coordinator. Other disciplines, such as speech-language pathology, dentistry or audiology, join on the team’s recommendation. Within 30 days of enrollment the team consolidates those eight assessments into one plan of care “through team discussions and consensus of the entire interdisciplinary team”.

Eight clinicians, eight assessments, one person, inside a month. Each one of them wants the same history: the hospital admissions, the specialists’ findings, the medication list as it actually was before the organization took over, the imaging that explains the gait. When the records arrive one fax at a time the eight assessments are done from eight partial views, and the consensus meeting spends its time reconciling what each member was told. When the outside record arrives organized, before the first assessment, the team starts from the same page.

Start a conversation about your medical records

Send your details and the calendar opens right here. Book thirty minutes and we will walk a record through the view you would actually use, or skip the call and we will come back in writing.

  1. Send your detailsTwo minutes. Your organization, name, email and phone, and the time records cost you; the rest is optional and helps us reply with something specific.
  2. Pick a timeThirty minutes, live, over synthetic records in the shape your team works in. No slides, and no discovery call before the demo.

Who this is for, and who it is not

  • Coverage comes firstVivlio retrieves from providers, hospitals and health systems it can reach through national exchange networks and direct connections. Name the facilities your patients come from on the form and we check coverage before anyone spends thirty minutes on a demo.
  • You can say what the chasing costsThe form asks how many hours a week your team loses to chasing records. If nobody can put a number near it, the case for Vivlio is not there yet, and we would rather say so than sell you a demo.
  • Patients and files at volumeThis is built for intake that runs every day. For one record, once, your patient can request it from the provider directly, and that will be faster than going through us.
  • Treatment, with the paperwork doneProduction access needs an NPI, a declared purpose of use and a signed BAA. Nobody swipes a card onto live PHI.

If none of those describe you, say so in the last field and we will come back in writing instead of booking a call.

Three to five is plenty. We check coverage for them before the call, so nobody finds out afterwards.
