Referral packet

The set of documents a referral source sends a home health agency so care can start: the order, the face-to-face encounter note, the discharge summary, the history and physical, the medication list and the demographics. What arrives varies by source; what the agency needs does not.

Also home health referral packet, referral documents

“Referral packet” is the industry’s word, not a regulation’s. No rule lists its contents, which is why its contents vary. What the rules do fix is what the agency must have before it can bill: a certification that the patient is homebound and needs skilled care, a face-to-face encounter documented in the certifying practitioner’s record, orders, and a plan of care. The packet is the referral source’s attempt to send those, plus the clinical history the admitting nurse needs for the initial assessment visit.

A complete packet carries six things:

  • The order or referral for home health, signed by the physician or allowed practitioner.
  • The face-to-face encounter note, within 90 days before or 30 days after the start of care, related to the reason for home health.
  • The discharge summary, if the patient is coming from a hospital or facility.
  • The history and physical, so the admitting clinician is not reconstructing the diagnosis list from the patient’s memory.
  • The medication list at discharge, which the drug regimen review in the comprehensive assessment depends on.
  • Demographics and payer information, including the Medicare number and the primary and secondary diagnoses.

Hospitals are required to send “necessary medical information pertaining to the patient’s current course of illness and treatment” to the post-acute provider at discharge (42 CFR 482.43). What that obligation produces in practice is a fax of whatever the discharge planner selected, and the face-to-face note and the history and physical are the two pieces most often missing. The agency then chases them against a 48-hour clock, or admits without them.

An outside-records query returns the hospital’s documents directly, from the hospital’s own system, whether or not the discharge planner included them. That is the difference between a packet and a record.

Start a conversation about your medical records

Send your details and the calendar opens right here. Book thirty minutes and we will walk a record through the view you would actually use, or skip the call and we will come back in writing.

  1. Send your detailsTwo minutes. Your organization, name, email and phone, and the time records cost you; the rest is optional and helps us reply with something specific.
  2. Pick a timeThirty minutes, live, over synthetic records in the shape your team works in. No slides, and no discovery call before the demo.

Who this is for, and who it is not

  • Coverage comes firstVivlio retrieves from providers, hospitals and health systems it can reach through national exchange networks and direct connections. Name the facilities your patients come from on the form and we check coverage before anyone spends thirty minutes on a demo.
  • You can say what the chasing costsThe form asks how many hours a week your team loses to chasing records. If nobody can put a number near it, the case for Vivlio is not there yet, and we would rather say so than sell you a demo.
  • Patients and files at volumeThis is built for intake that runs every day. For one record, once, your patient can request it from the provider directly, and that will be faster than going through us.
  • Treatment, with the paperwork doneProduction access needs an NPI, a declared purpose of use and a signed BAA. Nobody swipes a card onto live PHI.

If none of those describe you, say so in the last field and we will come back in writing instead of booking a call.

Three to five is plenty. We check coverage for them before the call, so nobody finds out afterwards.
