Also SOC, start-of-care visit, start-of-care date
The start of care is a date before it is anything else. It is the day the agency delivers its first billable visit and the day from which the regulations count. The initial assessment visit must happen within 48 hours of the referral or of the patient’s return home, or on the physician-ordered start-of-care date; the comprehensive assessment must be complete within five calendar days after the start of care; and the certification period of sixty days runs from it.
It is also a data point. The Start of Care OASIS is the first of the assessment time points, and the episode’s payment grouping and the agency’s publicly reported quality measures begin with what is recorded on it. CMS’s Timely Initiation of Care measure asks whether the start of care fell within two days of the referral or the hospital discharge.
What is often not in hand on that day is the patient’s history. The admitting nurse completes a drug regimen review with the medication list the patient can produce, records diagnoses from the referral and the patient’s account, and plans care from what the referral packet carried. Anything the hospital, the specialist or the prior physician holds and did not send is absent from the record the episode is built on, unless the agency went and got it first.
