2026 GACCP Fall Meeting · Peachtree City, GA
Outside records, ready before the first home visit.
A referral came in this morning. Before the nurse's first home visit, your intake team needs the diagnosis documentation, the history and physical and the medication list, and they sit at four offices that have not answered. This page is the walkthrough from the table: how that record arrives and sorts itself in minutes, and what it takes to run it on your own referrals.
Why we are here
The first visit waits on the record.
GACCP’s members run Georgia’s home and community-based services: CCSP and SOURCE, ICWP, NOW and COMP, and GAPP. Every one of those services starts the same way. A family calls, or a hospital discharge planner does, and before the nurse’s first home visit someone on your intake team has to get the history in hand: the diagnosis documentation, the most recent history and physical, the medication list from every prescriber, what happened in the hospital this spring. That history was written at four or five offices, and it arrives by fax, by portal and by phone, over weeks, in whatever order the offices answer.
Vivlio retrieves the outside record from providers, hospitals and health systems in minutes and organizes it for that first visit: what has arrived, from where, what is new, and who said it. The nurse walks in with the history. The care coach builds the plan of care from what has been diagnosed and prescribed, not from what the family remembers.
GAPP providers are in the same position with a different patient. The Georgia Pediatric Program pays for medically necessary in-home skilled nursing and personal care for children under 21 with significant medical needs, so that a medically fragile child is cared for at home rather than in a hospital. The nurse who starts that care needs the physician’s orders, the diagnoses, the hospital and specialist history and the supporting records behind them, the same documents the program’s service authorization asks for. They sit at the children’s hospital, the pediatrician’s office and the specialists, and the child is at home waiting. Vivlio retrieves them in minutes and organizes them for the start of care, so the plan of care is built from what has been ordered and diagnosed rather than from the packet that arrived last.
Three days from now, on Friday, October 23 at 11:00 AM ET, we are running a free live webinar on the same problem, the one advertised in GACCP’s Care Connections newsletter: Removing the biggest bottleneck in Medicaid waivers: Patient Records. Attend live and you are entered to win a $500 gift card.
The distinction
Hundreds of pages, or one record.
The files an intake actually receives, before and after Vivlio organizes them: one record per event, duplicates folded into their originals, and every line carrying its source and date.
What arrives today: eight files, three of them duplicates, in no order. What a home visit needs: five sections, duplicates removed, a source and a date on every line.
What arrives
Organized for the first visit, not just retrieved.
Retrieval is the commodity. What arrives has already been sorted for the visit ahead, and each line says where it came from.

The demo
This is what arrives.
The portal itself, rendered from the product's own source over a synthetic patient. Every line carries the organization that released it and the date it arrived. This is the view we walk through at the table.
Vivlio portal, release 5.30.0, rendered from source · Synthetic patient, no PHI

In minutes
The manual way isn't the only way anymore.
Vivlio retrieves and organizes the outside record in minutes. The manual way is intake staff on the phone chasing records, and a first visit that waits for them.

Questions
What people ask at the table.
- Was the demo at the table run on real patient data?
- No. Every walkthrough runs on synthetic records that say so on the screen. Production access to real records requires an NPI, a declared purpose of use and a signed BAA.
- Can the nurse see the records before the first home visit?
- Yes. Records arrive in minutes once requested, and every entry names the organization that released it and when. The intake coordinator can see which sources have returned and which have not before the visit is scheduled, so the nurse walks in with the history rather than collecting it at the kitchen table.
- What does it take to get started after the meeting?
- A thirty-minute call, the NPI of the ordering clinician, a declared purpose of use and a signed BAA. Leave your details below, or book the call from the calendar that opens, and we will walk one of your own referrals through the view.
Handling
What we do with records, stated plainly.
Built for real-world workflows, Vivlio supports the needs of both clinical and administrative teams while meeting strict privacy, security, and interoperability standards.
Production access is gated, by design
Real records require an NPI, a declared purpose of use, and a signed BAA. Nobody swipes a card onto live PHI.
HITRUST certification underway
Vivlio's HITRUST assessment is underway and on track to complete in November 2026: the certification healthcare security teams ask for by name.
Provenance travels with the record
Each line carries the source it came from and when it arrived, because a clinical decision needs to know who said it.
Built to WCAG 2.1 AA
Contrast, focus order and heading structure are requirements here, not a remediation project after procurement asks.

Leave your details from the 2026 GACCP Fall Meeting
Fill it in here on the tablet, or on your own phone. We follow up in writing after the meeting, or pick a time on the calendar for the week after and we will walk one of your own referrals through the view you saw at the table.
- Send your detailsTwo minutes. Your organization, name, email and phone, and the time records cost you; the rest is optional and helps us reply with something specific.
- Pick a timeThirty minutes, live, over synthetic records in the shape your team works in. No slides, and no discovery call before the demo.
Who this is for, and who it is not
- Coverage comes firstVivlio retrieves from providers, hospitals and health systems it can reach through national exchange networks and direct connections. Name the facilities your patients come from on the form and we check coverage before anyone spends thirty minutes on a demo.
- You can say what the chasing costsThe form asks how many hours a week your team loses to chasing records. If nobody can put a number near it, the case for Vivlio is not there yet, and we would rather say so than sell you a demo.
- Patients and files at volumeThis is built for intake that runs every day. For one record, once, your patient can request it from the provider directly, and that will be faster than going through us.
- Treatment, with the paperwork doneProduction access needs an NPI, a declared purpose of use and a signed BAA. Nobody swipes a card onto live PHI.
If none of those describe you, say so in the last field and we will come back in writing instead of booking a call.

We will come back in writing
Thanks - that is a useful answer. Before either of us spends thirty minutes, we will come back by email with the two or three questions that size what record chasing is actually costing your team, and what we can reach for the facilities you named. If the numbers are there, we will send a time then.
If you would rather just talk, the sales line below reaches the same people.
